Adult Deed Poll An Adult Deed Poll document can be used to change any part of your name i.e. change your forename, surname, add or remove name(s) or rearrange your current name. You must be at least 16 years old to change your name with a Deed Poll document.
Child Deed Poll As a parent or legal guardian you can change any part of your child's name with a child Deed Poll document. You must have consent from everyone with parental responsibility for the child.
